EDUCATION REQUIRED:

• Doctoral Degree in Medicine (MD or DO) required.

• Partners with administrative dyad and divisional/area/service line leadership to lead implementation and subsequent refinement of the H@H strategy in new markets and facilities.
• Collaborates closely with market and facility clinical and administrative leaders to ensure alignment of the Hospital at Home strategy with local goals and priorities.
• Ensures strategic alignment with other Continuing Health teams and strategies.
• Partners with the MIH Medical Director and administrative leadership along with Continuing Health Leadership Team to ensure strategic alignment of Hospital at Home and Mobile Integrated Health.
• Represents Advocate Health Hospital at Home at appropriate national and international conferences/meetings, within industry-specific collaborative teams, and in external advocacy efforts.
• In partnership with administrative dyad, designs, maintains, and optimizes clinical protocols and workflows that support the best outcomes for H@H patients across the Advocate Health enterprise.
• Responsible for achieving all key H@H performance indicators including quality, safety goals and other strategic outcome measures across the Advocate Health enterprise.
• Partners with the market-based H@H teams to grow the H@H referral base and overall volumes of the program in partnership with the appropriate Division, market and acute care facility leaders as well as other key stakeholders.
• Partners with market-based H@H Medical Directors to oversee and ensure optimal clinical performance of H@H physicians and APPs.
• Directly works with the appropriate Advocate Health Hospital Medicine leadership teams to address specific clinical concerns while simultaneously maintaining close partnership and communication on those clinical topics with the Senior Medical Director, Continuing Health.
• Partners with the market-based H@H teams to ensure Hospital at Home programmatic standards and alignment are maintained across the enterprise.
• Collaborates with the broader AH Continuing Health team to optimize the alignment of the AH Hospital at Home strategy with the other Continuing Health teams to connect patients to the right provider and the right program at the right time.
