About the Role: We are seeking a dedicated and compassionate Visiting RPN interested in a Full Time Permanent opportunity to provide high-quality nursing care to clients in their homes within the Greater Hamilton and Burlington area. Location: Greater Hamilton and Burlington Region Position: Community-Based Care Schedule: Weekdays and Weekends Day Shifts Vary: 7:00am to 3:00pm, 8:00am to 4:00pm, 8:30am to 4:30am Evenings: 3:00pm to 11:00pm On-Call Rotation Key Responsibilities: Participate in on-call rotations as required (typically twice per month overnights) Conduct in-home assessments and deliver nursing care based on individualized care plans Administer medications, wound care, IV therapy, palliative, pediatric care, and other clinical interventions Monitor patient progress and update care plans accordingly Educate patients and caregivers on disease management, medication adherence, and health promotion. Qualifications Education & Certification: Graduate from an accredited School of Nursing (RPN) Current registration in good standing with the provincial regulatory body LEAP Training, Palliative Certification, considered an asset Experience: Minimum of 2 years of nursing experience, preferably in ER or as a Visiting Nurse Strong clinical assessment and critical thinking skills Excellent communication and interpersonal abilities Proficiency with electronic charting systems Other Requirements: Valid G2 or G Driver’s license and access to a reliable vehicle Willingness to travel within the Greater Hamilton/Burlington area in all weather conditions We are grateful for all applicants.
