OVERVIEW:
The Hospice Intake RN is responsible for receiving information from incoming calls and coordinating clinical needs in the Hospice and Palliative Care department. This individual is responsible for coordinating information received with patients and families, referral sources as well as staff providing the service. This RN will answer inquiries for services, takes orders and facilitates the provision of services. This position refers to other interdisciplinary team members and community resources as necessary. QUALIFICATIONS:
Demonstrates excellent customer service Expands the visibility and understanding of VNA. Interprets insurance requirements for service coverage. Secures information necessary for care and coverage as needed. Represents agency to the public in a positive, cooperative manner. Acts as a liaison between physician and RN (verbal order; follow up calls). Participates in quality management programs involving data collection, analysis, reporting and trending to identify areas of need. Provides education to staff and other customers on Hospice and Palliative Care requirements and options as requested. Assess and screens patient/family needs and provide the patient/family with appropriate information. Chart review and patients contacts as needed. Provide triage guidance to patients based on current plan of care handling incoming clinical and non-clinical needs as appropriate including medication re-fills, scheduling, symptom management, determining PRN visit needs, etc. Exhibits excellent professional standards and utilizes critical thinking and skills while meeting the needs of Hospice and Palliative care patients, families and VNA staff. Conducts data collection for the continuous quality improvement program. Provides written and verbal reports on data collected as directed by manager. Assists with Plan of Care and HIS review as directed by leadership. Assists with coordination of patient care with other members of the team. Provides problem solving skills to assist in coordination of daily scheduling of HHA, SW, SN and Palliative Care clinicians. Participates in patient care conferences and team meetings allowing for the exchange of information in both formal and informal networking systems. Point of contact for volunteers providing patient care and is responsible for initiation of volunteer care plan when appropriate. Tracking of death certificates. Scanning and validation of clinical documents. Palliative Care pre-authorization support for providers. Assist with patient visits/admissions and care as directed. Other duties as assigned
