Description:
We have an exciting opportunity on the Child and Adolescent Unit for a temporary post to support the Vulnerable Child and Young Person Wellbeing Specialist Nurse.
The role incorporates both clinical and administrative aspects, with patients’ who are admitted to the ward and the Emergency Department with mental health and / or behavioural concerns or any other reason that may deem them a vulnerable CYP. You will have the opportunity to learn new skills by working alongside the Vulnerable CYP and Wellbeing Specialist nurse and the MDT, supporting with audits, communicating with patients, families and other health professionals, to improve the communication, support and overall experience of children and young people. The postholder will facilitate and support links with the Urgent Support Team, the Eating Disorder Team within CAMHS, and the Safeguarding Team here at Leighton.
The post holder will require good communication skills in order to support staff with plans made by the Multidisciplinary Team and the specialist Nurse and Consultants. We are looking for someone with an interest in Mental Health in children and young people who are looking to make a difference to an extremely vulnerable cohort of patients, reduce health inequalities and support CYP with a positive outlook for the future.
The post holder will work alongside the Vulnerable Children and Young People’s Specialist Nurse, to support the appropriate care pathways for vulnerable children and young people presenting with CAMHS and social care related issues. The post holder will be completing individualised risk assessments as guided by the specialist nurse and be able to escalate concerns for patient’s welfare accordingly, with the aim of improving their health outcomes and reduce health inequalities for vulnerable CYP.
You will work collaboratively with the multi-disciplinary team and contribute to the service provision through flexible working across patient pathways and contribute to the delivery of a developing service. You will have extensive knowledge of NHS policies and procedures relevant to the role and be able to demonstrate a range of skills required for the post including co-ordinating MDT meetings, and supporting the MDT in providing information to families and signposting to relevant services to meet the needs of the vulnerable CYP. You will be able to reflect on your practice and ensure that you deliver quality care.

1. Support the Vulnerable CYP Specialist nurse in completing and updating patient passports to ensure that an agreed care pathway is followed
2. Act as a point of contact to coordinate services as part of the MDT in the absence of the specialist nurse
3. Demonstrate good organisational and communication skills whist displaying a high standard of professionalism at all times, with an awareness of the need for confidentiality.
4. Have a clear and good understanding of safeguarding children, working within Trust policy, and completion of relevant training at Level 3.
5. Undertake patient specific training relevant to the role within the specialist service provision ( UST/CAMHS)
6. Demonstrate the ability to apply CPI techniques and interventions in practice to support patient care, improve patient and staff safety and be able to competently use de-escalation techniques as required.
7. Liaise with the multi-disciplinary team alongside or in the absence of the specialist nurse, to include ( but not limited to) the Urgent support team, safeguarding team, CAMHS, nursing and medical teams.
8. Work closely with patients and families to support their inpatient journey, whilst recognising the need to escalate to the VCYP specialist nurse or other professionals as required in a timey and appropriate manner.
9. Demonstrate excellent communication and interpersonal skills
10. Work using own initiative to ensure a smooth delivery of care and optimum patient experience
11. Demonstrate a good level of administration and clerical skill by being able to produce documents. spreadsheets and work with databases.
12. Be adaptable and able to prioritise workload when under presence and during periods of annual leave or sickness
13 Maintain own awareness of professional issues within an agreed framework as outlined by the Vulnerable CYP specialist nurse
14. Awareness of local and national drivers to guide and support the delivery of care to children and young people
15: Ensure compliance with Trust policies, procedures and guidelines for self and others, by taking action/alerting senior management team if practice appears to contravene policy, or if concerned about any aspect of patient care
