Join Waypoint as a Full Time Rapid Rehousing (RRH) Care Coordinator and become part of our Homeless Youth & Young Adult Continuum serving Rochester, NH. This role is perfect for someone who is organized, creative, compassionate, and committed to supporting young adults experiencing homelessness. You’ll work directly with youth ages 18–24, helping them move from homelessness to safe, stable housing and long-term success. What You’ll Do: * Assist youth in finding apartments and completing lease-up processes * Build positive relationships with landlords * Provide direct support, goal planning, and advocacy * Maintain program documentation and ensure compliance You’re a great fit if you: * Bachelor’s degree or equivalent experience (including certifications or lived experience) * Communicate effectively and build trust with youth * Are organized, adaptable, and self-motivated * Practice harm reduction, trauma-informed, person-centered care Why Waypoint?
