Position: Licensed Vocational Nurse (LVN/LPN) Reports To: Director of Patient Care Services Schedule: Thursday – Saturday, Day and Graveyard available About the Role As a Licensed Vocational Nurse at Cabrillo Hospice, you will provide compassionate, direct patient care under the supervision of a registered nurse. You will follow individualized care plans, administer treatments, and collaborate with our interdisciplinary team to ensure optimal patient outcomes. Key Responsibilities Deliver direct patient care in accordance with the State Nurse Practice Act. Implement nursing practices based on comprehensive patient assessments and care plans. Document patient care accurately and promptly. Provide patient and family education regarding diagnoses, treatments, and self-care. Coordinate with hospice services, including aides, OT, PT, MSW, ST, dietitians, and supervisors. Use equipment and supplies efficiently and effectively. Manage pain and other symptoms, evaluating patient responses to treatments and medications. Participate in personal and professional growth, quality improvement initiatives, and team development. Perform other duties as assigned by the supervising registered nurse. Qualifications Graduate of an accredited practical or vocational nursing program. Minimum of 3 years nursing experience; hospice, community health, or medical/surgical experience preferred. Current LPN/LVN license in the state. Committed to professional standards and nursing best practices. Strong communication, organizational, and interpersonal skills. Valid driver’s license with insured, reliable vehicle. Current CPR certification. Understanding of hospice philosophy and the needs of terminally ill patients. Why Join Cabrillo Hospice?
