Responsibilities * Provide compassionate care focused on comfort and quality of life for patients nearing the end of life. * Develop, implement, and manage individualized patient care plans under hospice protocols. * Conduct regular patient visits according to the established schedule, assessing patient needs and adjusting care plans. * Educate patients and families on end-of-life care, disease progression, and available resources. * Coordinate with interdisciplinary hospice team members, including social workers, chaplains, and therapists, to deliver comprehensive patient-centered care. * Administer medications and treatments as prescribed, monitoring for effectiveness and managing symptoms. * Document patient progress, changes in condition, and any interventions in the medical record promptly and accurately. * Communicate proactively with patients, families, and the care team to address concerns and provide emotional support. * Participate in hospice team meetings and contribute to developing patient care strategies. * Comply with all state, federal, and regulatory guidelines related to hospice care. Requirements * Active and valid RN license in the state of Ohio. * Minimum of two years of nursing experience required. * Prior experience in Med/Surg, Home Health, or Hospice settings is preferred. * Demonstrated understanding of hospice philosophy and end-of-life care. * Strong communication skills and ability to engage effectively with patients, families, and colleagues. * Valid Ohio driver’s license and reliable transportation for patient visits. * Ability to work independently and manage multiple priorities. * Comfort working with patients experiencing terminal illnesses and providing emotional support to their families. * Ability to pass a pre-employment background check * Ability to pass a pre-employment drug screen Life Enriching Communities is an equal-opportunity employer committed to workplace diversity and compliance with the applicable provisions of the Americans with Disabilities Act.
