Description

Ahanacare is looking for an experienced and exceptional Welfare Support Coordinator/ Intake officer on a FULL-TIME basis to join our team at Morley, Western Australia, reporting to the Service Manager or Director.

ESSENTIAL SKILLS AND QUALIFICATIONS

· Current First Aid certification
· Relevant advanced diploma or higher qualification in community service or community development
· Member of ACWA is desirable
· Demonstrated knowledge and skills in assessment, treatment and evaluation
· Demonstrated ability to work effectively in a multidisciplinary team setting as well as autonomously
· Ability to work with people from diverse backgrounds with compassion and empathy and without judgement for their lifestyle choices, personal preferences and values
· Demonstrated experience and knowledge of NDIS and working within a legislative framework

Responsibilities

· Liaise with all members of the support/care team and provide coordinated care.
· Carry out comprehensive and accurate client assessments.
· Contribute to the development, implementation, monitoring and review of care plans to meet client’s care needs and to promote quality of life.
· Promote a consultative and collaborative approach to client care planning and quality-managed activities with external stakeholders such as welfare and community service agencies, private entities, and government bodies (including Service Australia and NDIS)
· Monitor and report on the client’s progress.
· Maintain appropriate records and documentation.
· Support and assist clients in life skills development, mental well-being and reaching individual goals in the community.
· Refer clients to appropriate agencies for additional help if necessary.
· Assist with the orientation of new staff and ongoing professional development of staff.
