This position assesses and prioritises patient welfare needs and provides direct assistance to patients of an older adult inpatient service. You will liaise with government and other services providers to provide necessary assistance, support individuals with their recovery goals and work in conjunction with a multidisciplinary team providing a comprehensive welfare service for patients.
