WCS Healthcare Partners is seeking a full time Wellness Coach, to assist a large social service organization located in the Bronx, NY. A qualified candidate would be developing meaningful relationships with patients and families by providing resources for supportive care and guidance. Temp to perm role!

JOB REQUIREMENTS:

• Bachelor’s degree required.
• 6 months to 1 year of experience in social services.
• Prior experience as a Case Manager, Wellness Coach, or Peer Specialist.
• Experience documenting progress notes.
• Bilingual in Spanish is a plus, but not required.
