POSITION SUMMARY
The primary purpose of your position is to plan, organize, develop and direct the overall operation of the Wellness Department and to ensure the delivery of the highest quality assisted living services focused on maximizing residents’ individual capabilities in accordance with current federal, state, and local standards, guidelines and regulations that govern our facility, Title 22, and as may be directed by the Executive Director.

· Must be able to move frequently throughout the workday.
· Must be able to cope with the mental and emotional stress of the position.
· Must be able to see and hear or use prosthetics that will enable the senses to function adequately to ensure that the requirements of this position can be fully met (e.g.: accurately read measurements on resident related equipment such as thermometers, monitors, gauges).
· Must be able to function independently, have personal integrity, flexibility and the ability to work effectively with residents, employees, visitors and support agencies.
· Must be in a good general health and demonstrate emotional stability.
· Must be able to relate and work with the disabled, ill, elderly, emotionally upset and at times, hostile people within the facility.
· Must be able to lift, push, pull and move a minimum of 50 pounds.
· Must be able to assist with the evacuation of residents.
SPECIFIC REQUIREMENTS
· Must be able to read, write, speak, and understand the English language.
· Be a minimum of 21 years of age and of good moral character.
· Must be able to obtain criminal history clearance.
· Must comply with all training requirements.
· Must not pose a direct threat to the health or safety of other individuals in the workplace.
· Must be in good standing with the Federal Government.
· Have an obligation to inform your supervisor if you are on any government exclusion list.
· Possess the ability to make independent decisions when circumstances warrant such action.
· Ability to be calm and level-headed in emergencies.
· Well groomed, professional and possess ability to work harmoniously with other personnel.
· Ability to seek out new methods and principles and be willing to incorporate them into existing admission practices.
· Follows written and oral instructions.
· Knowledgeable in the use of computers, e-mail, spreadsheets and document word processing.
· Maintain patience, tact, cheerful disposition and enthusiasm, as well as ability to handle residents, staff, and visitors, based on whatever maturity level they are currently functioning.
· Ability to travel for training and work related activities.
QUALIFICATIONS
· Education: Must possess a High School Diploma or equivalent. Must possess a current First Aid Certification. Must complete 10 hours of training within the first 30 days of employment, including DOJ Elder Abuse Training, and 4 hours of annual continuing education; 6 hours of Dementia training within 30 days of employment, and 8 hours of annual continuing education. Additionally, must complete Medication Tech Certification Training, which includes: 8 hours of training within the first two weeks of employment covering the nine medication topics IAW 1569.69; 8 hours of hands-on medication administration training; pass an examination covering the nine medication topics; and 4 hours of annual continuing education.
· Experience: Must have at least two years of experience providing residential care to the elderly; or equivalent education and or training. Must have at least one year(s) of experience as a supervisor in a healthcare setting; hospital (geriatrics), skilled nursing or assisted living facility.

ESSENTIAL FUNCTIONS AND RESPONSIBILITIES
· Ensure the CAPLICO Core Values are adhered to at all times.

· Ensure that all residents receive quality care and maximize resident’s individual capabilities.
· Ensure that resident’s rights are well established and maintained at all times.
· Ensure an environment of teamwork and cooperation between resident care department employees/other departments.
· Maintain the confidentiality of all pertinent resident care information to ensure resident rights are protected.
· Assist in the development and implementation of departmental policies and procedures.
· Assist the Wellness Director with ensuring effective, efficient and adequate staffing of the resident care department by creating a work schedule that meets the needs of residents 24/7, while being mindful of staffing budget.
· Assist the Wellness Director with ensuring that care interventions meet the personal, physical and cognitive needs of each resident as well as maximize his/her self-care capacities, identify, independence, choice and opportunity for social interaction.
· Ensure that all applicable Human Resources policies and procedures and State and Federal employment laws and guidelines are followed by all resident care department employees.
· Assist the Wellness Director with ensuring that all caregivers are appropriately oriented, onboarded, and trained, according to Facility policy and procedure.
· Assist the Wellness Director with ensuring all caregiving staff complete required training and in-services according to established timeframes.
· Assist the Wellness Director with ensuring all caregiving staff are routinely trained in skills relevant to proper resident care, and to routinely audit and assess such skills to ensure they meet facility standards for resident care.
· Assist the Wellness Director with ensuring licensed care staff keep licenses current and in good standing with respective licensing authority.
· Assist the Wellness Director with ensuring employee and resident health and safety programs are implemented to provide a safe environment.
· Actively maintain cohesive public and professional relationships with residents, families, physicians and the community.
· Maintain ongoing education as it relates to maintenance of your professional skills and, where applicable, licensure as a nursing professional.
· Work with Wellness Director and Executive Director on Wellness department budgeting and spend downs; ensure adherence to established department budget while also ensuring that quality of resident care is not affected.
· Work with Wellness Director and Executive Director to promptly investigate and properly document any allegations of abuse or neglect.
· Assist the Wellness Director with monitoring and reviewing resident incidents and identify patterns that may require preventive intervention.
· Assist the Wellness Director with ensuring all caregiving staff is properly educated on and utilizes infection control practices that protect staff, residents and families.
· Periodically visit all Caregiver shifts to monitor performance of staff and to ensure quality care is being provided to residents.
· Assume full responsibility for the Wellness Department in the temporary absence of the Wellness Director or as directed by the Executive Director.
· Communicate resident care and service needs regularly to family.
· Assist Wellness Director with completing resident assessments to determine level of care and appropriate placement.
· Assist with new resident orientation and transition to community.
· Meet with residents regularly to encourage communication and gain understanding of their needs.
· Conduct daily rounds to review performance of care staff.
· Schedule ancillary services: podiatry, dentist, therapies, pharmacy consultation, etc., as requested by the Wellness Director.
· Continuously prepare and ensure readiness for state survey; conduct follow-up to ensure corrective actions are appropriately completed.
· Ensure documentation of resident clinical status, ensure communication to physicians and other healthcare professionals are documented in resident record.
· Provide hands-on care as staffing needs require (and licensure allows).
Communicate and follow-up with provider pharmacy to ensure compliance with resident medication orders.
· Assist Wellness Director with conducting audits of medication records and med storage areas.
· Performs other duties as deemed necessary and appropriate, or as may be directed by the Wellness Director or Executive Director.
SUPERVISORY RESPONSIBILITIES
· Exercises administrative and functional supervision over all Wellness Department employees in the absence of the Wellness Director, as delegated.
