Reports To: COIPP Program Manager FLSA Status: Non-Exempt Position Function: Non-Clinical Classification: Regular, Part-time Schedule: 20 Hours/Week: Wednesday 11am-3pm/Thursday-Friday 8am-4:30pm (some weekends/evening as needed) Location: Onsite Grant End Date: 01/31/2030 SUMMARY: Under the direction and supervision of the COIPP Program Manager, this position is responsible for assisting families in helping at risk individuals get treatment and find hope toward recovery. Education and Outreach while promoting programs and teaching tribal communities and families about opioid use and addiction is a key objective for the Wellness Navigator. This position will also require identifying Recovery Champions in the community to share their lived experience to inspire recovery efforts and celebrate sobriety. ESSENTIAL DUTIES AND RESPONSIBILITIES: The information in this job description indicates the general nature and level of work performed. Facilitates interdepartmental meetings, ensuring information is disseminated and gathered with accuracy and cultural relevancy. 2. Clearly communicates project goals and under the direction of the COIPP Program Manager. 3. Coordinates appropriate activities/meetings to ensure goals are met. 4. Serves as a navigator to facilitate the gathering of intake and subsequent data required by funding source and determine readiness of program. 5. Works closely with coalition/advisory members, the treatment team, and outside entities to enhance the development and implementation of the community outreach and community agencies. 6. Promotes community wellness through involvement in cultural and social activities known to support individual and community resilience. 7. Maintains well-organized documentation, tracking, and reporting processes. 8. Completes project reports in a timely fashion. Completes all data entry for grant funder. 9. Completes all reporting for grant reporting based on the Notice of Award. 10. Collaborates with other IHC department staff. 11. Attends department meetings, provides presentations to staff and program affiliates. 12. Participates in local community gatherings/events for outreach and recruitment. 13. Serves as Peer Support for patients and community members in COIPP Programming. 14. Complete all grant objectives based on Narrative. 15. Assists with various group workshops as planned by the Health Promotions Department. 16. Other duties as assigned. 17. Position may be re-assigned during activation of EOP. SUPERVISORY RESPONSIBILITIES This position has no supervisory responsibilities. TRAVEL & DRIVING REQUIREMENTS · Travel is not required for this position · Driving is required for this position EDUCATION AND/OR EXPERIENCE · High School Diploma or GED · Two years related community outreach and education experience, and/or training. REQUIRED KNOWLEDGE, SKILLS, AND ABILITIES · Experience working with Tribal communities. · Knowledgeable of Southern California Tribes. · Knowledge of consortium community and resources as it relates to position. · Ability to interpret and translate grant goals and objectives into effective services for the community. · Ability to work effectively with team members. · Ability to identify problems, use good judgment and recommend solutions. · Ability to establish priorities and coordinate work activities. · Possess excellent interpersonal communication skills. · Possess excellent public speaking skills. CERTIFICATES, LICENSES, MEMBERSHIPS AND REGULATIONS · A valid California driver’s license and a good driving record will be required at the time of appointment and must be maintained throughout employment. · Current CPR certification or attainment within 6 months of hire. · Applicants must successfully pass a pre-employment physical exam, tuberculin skin test or x-ray and urine drug screen test that includes marijuana. · Health must be adequate to perform all duties of the position. · Must pass a criminal background check.
