Job Purpose:
The Wholeistic™ Health Coach will be responsible for providing virtual health coaching to employees participating in an employer sponsored wellness program. Using technology, you will provide health and wellness coaching to individuals to improve their health status and reduce risk factors from a home office or remote location.
Wellness services are provided within the boundaries of the State Nurse Practice Act, other legal and regulatory guidelines as well as Company policies.
Ability to work in a matrix environment and maintain a high degree of confidentiality and discretion.
This person must have an interest in interacting respectfully with employees and spouses to promote general health by emphasizing the principles of screening, prevention and health consumerism.
Duties - Under the direction of the Coordinator’s assigned location’s Human Resources Manager and US HealthCenter:
· Provide health counseling to reduce participant risk factors for heart disease, diabetes, cancer, and other diseases and conditions as well as improve lifestyles and total wellbeing.
· Assist participants in creating a long-term vision for their health and wellness.
· Set goals with participants focusing on exercise, nutrition, weight loss, stress management, alcohol, tobacco, medication adherence, disease management, disease prevention, and following health guidelines.
· Interpret data from wearable devices and in-home monitoring equipment to track patient biomarkers, such as blood pressure or glucose levels.
· Create a link between the participant’s health status and the employer’s benefit offerings. Teach participants how to utilize benefit plan and health related tools and services.
· Guide and increase the participant’s use of coaching, online tools, physician engagement, and services.
· Answer all questions related to incentive program offerings and how they relate to benefit offerings.
· Be an overall health resource for the participant, their family, and their interaction with the health care delivery process.
· Engage in dialogue with participant’s caregivers, providing advice and steerage to appropriate care.
· Develop a relationship that creates an ongoing and in depth engagement with the member leading to measureable risk reduction and self stewardship of health management.
· Maintain accurate and confidential patient records using electronic health record (EHR) systems.
· Perform other duties as directed and assigned by management.
Skills/Qualifications:
· Bachelor of Science in Nursing required.
· Active Registered Nurse (RN) license in the State of California required. For roles serving patients in multiple states, a multi-state license through the Nurse Licensure Compact (NLC) may be required.
· Knowledge of disease prevention and principles of a healthy lifestyle (including exercise, nutrition, weight management, and stress management, routine preventive health screenings and disease management).
· Minimum five years’ experience in direct education and/or coaching individuals for disease prevention, health promotion, and/or behavior change.
· Knowledge and experience in complex or case management a plus.
· Ability to complete our Health and Wellness Coach Certification program within one year of hire.
· Proficiency in Microsoft Office (Word, Excel, Outlook), Internet, telehealth platforms, remote monitoring technology, and database documentation.
· Excellent oral communication skills.
· Self-starter able to self-direct work.
· Laptop and personal phone required or provided.
