Our Values R – Respect I – Innovation S – Stewardship E – Excellence Women’s Health Registered Nurse (RN) Summary Provides patient-centered prenatal, postpartum, and newborn care coordination, including tracking high-risk referrals (e.g., Maternal-Fetal Medicine and abnormal genetic testing), ensuring timely follow-up, and offering education and support within RN scope. The Women’s Health RN will work within a multidisciplinary care team that takes an integrated approach to maternal and newborn health, spanning from pregnancy through childbirth, and postpartum. Care Management · Tracks high risk patients including Maternal-Fetal Medicine (MFM) patients from the time of referral to ensure they follow up with the MFM provider, complete recommended treatment, and return to PHS for follow-up care. · Care coordination for abnormal genetic testing results to ensure high-risk patients are following recommendations and returning to PHS for continued care. · Coordinates prenatal and postpartum appointments to ensure continuity and timely access to care. · Monitoring postpartum patients and ensuring they are scheduled for their follow-up visit at PHS and follow-up outreach to postpartum patients to ensure timely follow-up care. · Assisting parents/guardians with getting their newborn scheduled for their first visit. · Provides prenatal and postpartum education and support, including topics such as breastfeeding, nutrition, warning signs, mental health, newborn care, and family planning. · Assist patients in understanding and navigating their care plans, referrals, and follow-up appointments. · Connect patients with other PHS services such as behavioral health, dental, primary care or WIC. · Supports patients in developing individualized health goals and care plans in collaboration with the provider and interdisciplinary team. · Collaborates with care coordinators and community partners to address social determinants of health impacting maternal and infant well-being. · Facilitates scheduling of diagnostic tests, ultrasounds, lab work, and specialist referrals. · Assists in tracking follow-up care and ensuring that patients complete recommended visits and screenings. Clinical: · Administers injections, vaccinations and medications per provider direction. · Monitors and documents vital signs, patient assessments, and responses to treatment. · Assists providers with in-office gynecologic and obstetric procedures (e.g., Pap smears, IUD insertion/removal, biopsies, specimen collection, Etc.). · Maintains readiness of exam and procedure rooms; ensures instruments and supplies are prepared and sterilized according to clinical protocols. · Serves as a clinical resource and mentor for medical assistants and nursing team members in the Women’s Health department. · Participates in staff training related to women’s health procedures, prenatal/postpartum care, patient communication, and documentation best practices. · Supports onboarding of new staff to Women’s Health workflows. · Supports accurate and timely documentation in the EMR system. Other: · Properly handles the disposal of infectious/hazardous waste, according to OSHA guidelines. · Inventories and requests supplies. · Responsible for performing health and team-based care related services within the guidelines of the state of Ohio Board of Nursing. · Support a shared goal model in order to achieve high-quality care that is patient and family centered. · Participates in care team huddles and case reviews to identify and proactively address patient needs. · Participate in team meetings and quality improvement activities as necessary. · Performs all other duties and tasks as assigned. Bachelor’s program preferred. Language Skills Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Reasoning Ability Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Computer Skills To perform this job successfully, an individual should have the ability to gain knowledge of current practice management system, electronic medical record, Microsoft Word, text paging, Internet, and Intranet. Certificates, Licenses, Registrations RN License. Must hold a current, valid license to practice nursing in the state of Ohio. Other Applicable Requirements Ability to speak Spanish desirable. Knowledge of examination, diagnostic, and treatment room procedures. Knowledge of medical equipment and instruments to administer patient care. Skill in taking vital signs. Skill in maintaining records and recording test results. Skill with geriatric patients and patients in lower socio-economic sectors of the community.
