Franciscan Health Olympia Fields Campus 20201 S Crawford Ave Olympia Fields, Illinois 60461 The Wound Care and Enterostomal Specialist RN is responsible for providing total comprehensive nursing to patients and their families through the development of a plan of care, including implementing, reassessing, evaluating and revising the plan of care. The RN develops these patient-specific plans by performing psychosocial/physiologic assessments, considering patients' cultural/ ethical/ spiritual needs, reviewing trends of similar patients, as well as considering physicians' orders regarding care. The RN integrates knowledge, skills, and experiences to meet the needs of patients and families throughout the continuum of care to include patient and family education, and actively supports and incorporates nursing regulations and standards, as well as Franciscan Alliance processes and procedures in daily practice. This position has expanded knowledge and certification in wound care, continence, and enterostomal therapy and as such, delivers wound, skin, enterostomal therapy and ostomy nursing care. The RN develops programs for the education of patients, families, and staff within the specialty area. WHAT YOU CAN EXPECT Current Openings for Part-Time PRN Days Care Coordination Facilitation Facilitates the coordination of the multidisciplinary delivery of patient care. Stoma Measurement Visits patients' pre and post operatively to assess for stoma measurement for placement, stoma care needs and to teach stoma self-care. Provides consultation services for unresolved patient care issues. Arranges for stoma and ostomy care equipment and supplies for discharge planning. Sharp Debridement Performs conservative sharp debridement, defined as removal of loose avascular tissue without pain or bleeding. Skin Breakdown Risk Assessment Identifies high-risk patients in the facility for skin breakdown and serves as a liaison to the physician and nursing staff to initiate a plan of care with appropriate measures for prevention. Nursing Practice / Standards of Practice Practices nursing according to Franciscan Alliance and nursing division's philosophies and standards of care set by state board of nursing, state nurse practice act, and other governing agency regulations. Evaluates one's own nursing practice in relation to professional practice standards and guidelines, relevant statutes, rules, and regulations. Compassionate Care Establishes a compassionate environment by providing emotional, psychological, and spiritual support to patients, friends, families, and coworkers. Collaboration & Patient Experience Collaborates with patients, their families and primary caregivers as well as members of a multidisciplinary team, to ensure optimal patient outcomes and enhancement of the patient experience. Patient Assessment Assesses, collects, and analyzes comprehensive objective, subjective, psychosocial, and cultural data pertinent to patient condition to determine the nursing diagnosis, patient needs and plan of care. Responds to patient reassessments and responses to care plan. Plan of Care Development Develops an appropriate plan of care based on objective and subjective data, observations, and communications from patient and family and continuously reviews and revises plan of care as patient condition warrants. Plan of Care Implementation Prioritizes and implements the identified plan, coordinates care delivery, employs interventions to improve patient outcomes while providing a safe, respectful, and ethical practice environment. Plan of Care Evaluation Evaluates the individualized plan of care and progress towards attainment of outcomes in collaboration with patient/family utilizing available data/resources. Patient's Assessment/Ongoing Reassessment Communication Communicates timely changes in the patient's assessment and ongoing reassessment to providers that warrant consideration and/or a change in the plan of care. Maintains and coordinates communication with all members of the multidisciplinary team to facilitate safe transitions and continuity in care delivery. Maintains continuity among nursing teams by documenting the assessment, plan of care, interventions, and responses to care in the electronic medical record in a timely manner. Patient Education - Independence Promotes patient's independence by establishing patient care goals and teaching the patient, family and caregivers to understand the condition, medications, and self-care skills; provides support and answers questions. Safe Work Environment Maintains a clean and safe work environment by handling and clearing instruments, equipment, and supplies in accordance with infection-control policies and protocols. Administers and stores medication according to controlled substance regulations. Equipment & Supplies Maintenance Ensures operation of equipment by completing preventive maintenance requirements as per manufactures instruction or calling for repairs when required. Maintains nursing supplies inventory by checking stock to determine inventory level, anticipating needed supplies and placing orders for required inventory. Evaluation of New Equipment Facilitates the evaluation and implementation of new products and equipment related to the management of wounds based on most current evidence and standards of practice. Training & Education Participates in activities that contribute to ongoing development of professional knowledge of staff. Collaborates with the education department to identify staff education needs and in the provision of nursing staff orientation. QUALIFICATIONS Required: Bachelor's Degree in Nursing/Patient Care Preferred: Master's Degree Required: 3 years Nursing/Patient Care experience Preferred: 3 years Wound Care experience Required: Registered Nurse (RN) - State Licensing Board Illinois Required within 1-1.5 years: Wound Care Certification (WCC) - National Alliance of Wound Care and Ostomy OR Preferred: Certified Wound Ostomy Continence Nurse (CWOCN) certification - Wound, Ostomy and Continence Nursing Certification Board Required within 90 days: Basic Life Support Program (BLS) - American Heart Association Prime Healthcare will add the 214‑bed Franciscan Health Olympia Fields Hospital and Franciscan’s Illinois operations, including Specialty Physicians of Illinois LLC, Working Well’s Illinois operations, Franciscan Health & Fitness Center Chicago Heights, Home Care, and Chicago Heights Walk-In Clinic, to its system.
