Part-time (28 hours/week) Job Summary: We are seeking dedicated and compassionate Wound Care Nurses (LPN or RN) to join our growing healthcare team. Key Responsibilities Conduct thorough assessments of wounds and patients’ overall health status. Develop and implement individualized care plans. Perform wound care treatments including dressing changes and application of topical agents. Monitor healing progress, watching for infection or complications. Educate patients and families on wound care, prevention, and self-management. Collaborate with other healthcare professionals to coordinate care and optimize outcomes. Accurately document assessments, treatments, and patient progress. Provide care for all home health patients within scope of practice. Stay current with best practices and advancements in wound care. Requirements Qualifications Active nursing license (LPN or RN) in the state of practice. Wound care experience required (home health experience strongly preferred). Strong understanding of wound healing principles and infection prevention. Familiarity with DME (Durable Medical Equipment) related to wound care. Ability to travel between patient homes (reliable transportation required). Excellent communication and documentation skills. Compassion, flexibility, and a commitment to high-quality patient care. Additional experience with pediatric patients, tube feeding, or medical-surgical nursing is a plus. Must be willing to drive through a designated territory as well as have wound care experience.
