POSITION OVERVIEW:

We are seeking a Medicare-credentialed Nurse Practitioner (NP) with to join our team. This role involves providing mobile, advanced wound care treatments in various locations in Sacramento, CA , ensuring the highest level of patient care.

QUALIFICATIONS:

✅ Licensed Nurse Practitioner (NP) in California
✅ Willing to travel for training in Advanced Wound Care practices
✅ Medicare credentialed (required)
✅ Ability to travel throughout Sacramento, CA and surrounding areas
✅ Strong EMR skills and documentation accuracy
✅ Excellent communication, patient education, and bedside manner

Responsibilities
  • Provide specialized wound care treatments in patients’ homes, SNFs, and other facilities.
  • Assess, diagnose, and develop customized treatment plans for chronic wounds.
  • Perform advanced regenerative wound care (collagen, amniotic allographs & biologics).
  • Follow Elemental Living’s established wound care protocols and best practices to ensure consistency and high-quality patient care
  • Educate patients, caregivers, and facility staff on wound management and prevention.
  • Maintain accurate patient documentation and adhere to Medicare guidelines.
  • Collaborate with healthcare teams to optimize patient care and healing outcomes.
