OakLeaf Clinics - Wound Care has an exciting opportunity to join our expanding team as a full-time, 0.8 FTE Registered Nurse! OakLeaf Clinics is dedicated to providing our patients with compassion, trust, and a lifetime of individualized care. Our healthcare team consists of physicians, nurse practitioners, physician assistants, dietitians, nurses, respiratory therapists, and medical assistants working in concert with laboratory/imaging services to offer individualized care to the Chippewa Valley.  Position description Duties include rooming patients, taking vital signs, completing medication reconciliation, administering medications, including injections, scheduling appointments, completing required prior authorization paperwork, assisting providers with their day-to-day clinical practice, and completing other duties. Responsibilities * Assess the patient's needs to make appropriate and independent/collaborative decisions for selecting a specific triage plan * Use departmental evidence-based protocols to triage patients * Instruct patients on their healthcare * Document all interactions with patients and providers when medical direction is given * Coordinate services with other departments for follow-up based on the patient's needs * Communicate problems, concerns, and suggestions to lead nurses or the clinic manager * Maintain up-to-date knowledge and skills in professional and clinical areas * Provide professional nursing care under supervision or within the scope of licensure for clinic patients * Facilitate/perform the gathering of patient information and document accordingly * Schedule follow-up appointments * Instruct patients and families by phone as directed within the scope of licensure or certification * Screen patients for appropriate information * Maintain a timely flow of patients and complete forms as needed * Assist with office procedures as directed * Participate in professional development activities and maintain professional affiliations * Maintain patient confidentiality Qualifications * Graduate of an accredited school of nursing * Current State of Wisconsin Registered Nurse License or Nurse Compact License * Basic Life Support (BLS) certification * Medical terminology knowledge and understanding of patient care notes * Ability to demonstrate effective written and verbal communication skills to complete documentation for medical record purposes * Excellent interpersonal and customer service skills * Detail-oriented and accurate * Passionate about providing patient-focused care * Ability to multitask with attention to detail * Prior Epic experience preferred Work environment While performing the duties of this job, the employee regularly works in a clinic setting.
