Description Job Summary: The Wound Care Registered Nurse is responsible for providing and coordinating comprehensive care through the appropriate utilization of assessment, diagnosis, planning, implementation and evaluation, in accordance with established standards, policies and procedures. SHIFT: Monday-Friday - 9 a.m. - 5 p.m. On call every fourth weekend Learn about Plymouth Place Benefit Packet offered to all Full-Time Employees: 2026 Benefit Guide here ESSENTIAL JOB FUNCTIONS/RESPONSIBILITIES: Employee will apply skills to ensure Person Centered Care as exhibited through our Pillars of Excellence: PEOPLE STEWARDSHIP SERVICE QUALITY GROWTH · Provide excellent nursing care to residents suffering from wounds. · Assess and evaluate residents with wounds and injuries. · Obtain cultures to assess wounds and injuries. · Evaluate wounds and injuries for infections or other illness factors that causes wounds. · Initiate nursing care procedures in managing acute and traumatic wounds. · Consults with Nursing team concerning assessment evaluations and assist in planning a developing the skin care treatment to be performed. · Demonstrate wound care procedures to other caregivers, i.e., RNs and CNAs. · Assesses resident needs, and provides necessary nursing care as per resident's care plan. · Completes assigned MDS and participates in interdisciplinary care plan conferences. · Assures that resident rights are maintained. · Administers prescribed medications and treatments in accordance with approved nursing techniques. · Participates in on-call rotation as directed by Director of Nursing. · Assumes the role of shift supervisor as needed. · Works as floor nurse as needed. · Documents nursing history and physical assessment for assigned patients upon admission; develops interim plan of care. · Comply with applicable federal, state and local laws, rules, and regulations; maintains knowledge of and follows all Plymouth Place policies and procedures. · Maintain regular and punctual attendance at work and meetings. · Attends in-service and education programs. Requirements · Current Illinois Registered Nurse License in Illinois is required; Bachelor of Science in Nursing (BSN) recommended · Basic Life Support (CPR) · Certification in Wound Care recommended - Wound care certification must be completed within 3 months of hire · 2-3 years’ experience with wound care KNOWLEDGE, SKILLS AND ABILITIES REQUIRED: Knowledge of care of injuries and wound care. Experience in rounds with doctor for care and treatment. Proficient with Microsoft Office Suite or related software. Solid organizational skills including attention to detail and multi-tasking skills. Ability to read, write and speak English sufficient to perform job duties and to interact and communicate with residents, families, and employees.
