The Wraparound Care Coordinator works in collaboration with community partners and MCCFL Clinical Services Team to promote each client’s ability to effectively engage in services. With the high prevalence of trauma among individuals receiving mental health services, the Wraparound Care Coordinator must possess demonstrated understanding of the effects of trauma on health, relationships, coping and other aspects of the lives of those we serve. This role ensures that the Wraparound process is followed with fidelity and in accordance with the principals to empower youth and families to achieve their individualized goals. They initiate parent and youth engagement, support team development to include natural and professional supports, facilitate regular meetings, develop, and implement individualized family plans of care plans that reflect the youth and family’s prioritized needs and preferences. The Wraparound Care Coordinator will work with youth and families who are multi-system involved (juvenile justice, child welfare, substance abuse, special education, etc.) and must understand the complex systems of public services with the ability to forge strong connections to support client’s stabilization within their community. Clients and family units may present with a variety of mental health challenges; trauma history, emotional and/or behavioral dysregulation, suicidality, disruptive attachment, and relationship difficulties.
Mid-Columbia Center for Living (MCCFL) is a Certified Community Behavioral Health Clinic (CCBHC) that specializes in providing services that are both trauma-informed and recovery-oriented to persons/consumers with mental illness and addictions. Trauma-informed care includes safety, choice, collaboration, trustworthiness, cultural responsivity, and empowerment for both clients and staff. MCCFL serves Hood River, Wasco, and Sherman Counties and utilizes a consumer-involved, consumer-engaged, and consumer-driven model. MCCFL provides a whole-person care approach to adults and children to address both physical and mental health needs. MCCFL offers a variety of programs including a mobile crisis unit, intensive case management, ongoing counseling, community support, supported employment, jail diversion, hospital discharges, hearings, and more.
