Description General Description: The Housing Care Manager is responsible for providing comprehensive, trauma-informed services to individuals and families in the Pat’s Place Transitional Living Program (ages 18–24) and runaway and homeless youth under 18 in the River Haven Youth Shelter. Services include providing general program information, crisis intervention, intake and assessment, and the support of their comprehensive, goal-oriented service plans. The Housing Care Manager provides guidance, advocacy, and coordinated support to assist participants in achieving safety, stability, independence, and overall well-being, while ensuring compliance with program rules and applicable HUD and RHY regulations. Principal Duties: Provide general information about program expectations and support engagement of participants within both the transitional living and youth shelter programs. Support the implementation of service plans developed by reinforcing goals, expectations, and identified interventions during assigned shifts. Assist in maintaining a safe, structured, and supportive environment by providing supervision, guidance, and redirection appropriate to participant age and developmental level. Reinforce independent living skills and daily routines, including: hygiene, room maintenance, and daily responsibilities Time management, school/work attendance, and curfew compliance Provide support during crisis situations by utilizing de-escalation techniques, ensuring immediate safety, and communicating all incidents to supervisory and daytime staff. Observe, document, and communicate participant behaviors, progress, and concerns to ensure continuity of care and alignment with established service plans. Monitor participant compliance with program rules and expectations, and implement program-approved interventions and consequences as directed by supervisory or daytime Care Management staff. Assist participants in accessing scheduled services, appointments, and activities, and help reinforce follow-through with referrals made by daytime Care Managers. Support the facilitation of structured activities, life skills groups, and recreational programming as directed. Maintain accurate, timely, and objective documentation, including case notes, shift reports, and incident reports in accordance with program requirements. Communicate effectively with team members across shifts to ensure consistent implementation of service plans and participant support strategies. Maintain appropriate professional boundaries and model positive behavior and communication at all times. Assist with general program operations, including maintaining cleanliness and organization of common areas, as needed. Participate in staff meetings, trainings, and supervision as required. Immediately report safety concerns, incidents, or mandated reporting situations in accordance with agency policy and applicable regulations. Qualifications: Experience: Two years related work experience with youth population. Must have competent reading and writing skills along with strong communication skills. Mental health experience preferred. Education: BA/BS in human service field or equivalent combination of education, training and experience. Licensing/Certification Requirements: Valid NYS Driver’s License required. Must have a valid license to transport clients as necessary and/or to be able to get to the various worksites located throughout the agency. Proof a valid car insurance policy is necessary when required to use personal vehicle for travel. Physical Demands: Must be able to climb multiple sets of stairs. Must be able to lift a minimum of 25 pounds to assist with youth moving in and out of apartments.
