The role of a Mental Health Care Coordinator is to work with youth (ages 5-21) and their caregivers to identify needs and goals that will strengthen the family and the youth's future. Areas of focus include education, mental health, physical health, economic needs, and positive social development. Youth and families are met with weekly on average and work towards these goals together in the community where the needs are as well as in the family home itself. Staff transport clients when needed to make sure important connections to community resources are successful. The combined efforts of the care coordinator and the informed, client led approach, deliver some of the best outcomes in the field of human service work.
