EDUCATION, EXPERIENCE, OTHER QUALIFICATIONS:
Education Required: A Bachelor’s degree from an accredited college or university with a major in psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human growth and development, physician assistance, gerontology, special education, educational psychology, early childhood education or early childhood intervention or a bachelor’s degree with at least 30 hours of coursework in the previous fields.
Experience Required: Previous experience in the field of mental health or social services preferred.

POSITION SUMMARY/JOB PURPOSE:
The Youth Mental Health Case Manager provides evidence-based services to children and their families in maintaining or improving their ability to function in the community and work toward their recovery goals. This position assists youth with mental illness and/or behavioral issue and their families access medical, social, educational and other appropriate services. This position encourages youth to live their best lives through support, resources, skills training, parent training, safety planning, crisis intervention, and many other services. The Youth Mental Health Case Manager works in collaboration with other agencies within the community and state to link youth and their families to other needed services. This position provides monitoring, assessment, service planning and coordination, and skills training and development.
This position works independently, under limited supervision, reporting major activities through periodic meetings.
