ABOUT US:

Part of Lutheran Services, Intercept at Caboolture is a not-for-profit community organisation that provides early intervention and prevention services for young people and their families in the Moreton Bay region.
Intercept deliverers targeted support programs, unique to young people and their families’ individual circumstances, that support them to achieve their goals/improve quality of life. Underpinning our evidence-based service design are our Lutheran Services values and Model of Care and Service. Our team is warm and friendly and we live our values at work every day.

In this full-time role, you’ll provide one-on-one mentoring and small group support to young individuals, including those with disabilities, to build independence, interpersonal skills, and engagement in education and community.

• Deliver tailored mentoring sessions and small group interventions
• Support young people to set and achieve goals through strength-based approaches
• Work flexibly using outreach and community-based models
• Engage with families, schools, and local networks to create strong support systems
• Promote inclusion and capacity-building for clients with diverse needs
• Maintain accurate records and collaborate with team leaders and case managers
