About the Role
We’re looking for a passionate, proactive Case Manager to be a steady support and strong advocate for young people across Melbourne’s Western and Northern regions.
This is more than a job — it’s an opportunity to be a consistent and compassionate voice in someone’s journey toward independence and self-belief. You’ll be part of an experienced and dedicated Youth Services team delivering assertive outreach case management and holistic support to young people experiencing or at risk of homelessness.
Reporting to the Youth Services Coordinator, this is a fixed-term full-time position, contracted until October 2026, based in Sunshine, VIC.
Compensation is in accordance with SCHADS Award Level 5.

You Will Successfully

  • Help young people transition out of the homelessness system with dignity and confidence
  • Support youth in and transitioning from out-of-home care, working collaboratively with Child Protection and other program (e.g. Better Futures)
  • Provide trauma-informed, youth-focused intake and assessments via our access point
  • Collaborate with refuge staff at Tranmere Street to support daily operations and promote life skills development
  • Ensure all support aligns with our Model of Care and a Psychologically Informed Environment

You Will Have

  • A tertiary qualification in Social Work, Youth Work or a related field (degree preferred)
  • Hands-on experience working with at-risk or vulnerable youth, particularly in areas of homelessness, AOD, mental health or out-of-home care
  • A deep understanding of trauma-informed care, youth-centred practice, and social justice
  • A VIC Driver’s Licence
  • A current and valid Employee Working with Children’s Check
