Description

This position requires travel throughout Orange County. The home office will be located in the 32811 zip code. The hours will be Mon-Fri 10am-6pm.

QUALIFICATIONS:

Bachelor’s Degree in Social Work Criminal Justice, Human Services, or related field. Minimum 1-2 years working with youth in community or school based setting. Ability to engage and build rapport with youth and families. Strong organizational and documentation skills. Familiarity with community resources and systems of care in Orange County. Reliable transportation and willing to travel within Orange County as needed. Must have clean driving record, valid driver’s license, and pass Level II background screening.

Responsibilities
  • Facilitate individual and group programming on life skills, emotional regulation, conflict resolution and violence prevention for youth and youth adults ages 1325, residing in Orange County.
  • Conduct outreach and engagement with atrisk youth and families in home and community settings.
  • Provide trauma informed support services to parents and caregivers to promote positive parenting strategies.
  • Collaborate with multidisciplinary teams to include schools, law enforcement, behavioral health and community based organzations.
  • Provide referrals and coordinate care with external agencies as needed.
  • Maintain accurate documentation of client interactions in accordance with reporting requirements
  • Collect and report data for program evaluation, performance measurement, and grant compliance.
  • Attend and contribute to case staffings and community meetings.
