Adjust doses when allowed by Collaborative Practice Agreement, or provide recommendations to prescribers. Perform medication reconciliation to ensure patient’s medication list is as accurate as possible, particularly during transitions of care. Order appropriate lab tests to ensure adequate monitoring of medication therapies. Provide recommendations to help patients manage their out of pocket costs for medications. Provide patient education around medications, devices, and disease states. Quality Improvement Participate in projects to improve the medication use process at the clinic and health system in general. Provide formal and informal training to other staff to improve their medication-related knowledge. Assist in developing and revising standards of care for the MTM Pharmacy team. Collaborate with clinic staff to improve workflows and address site-specific needs. Participate in appropriate meetings within the health system.
