As part of a new operating model designed to maximise the independence of adults in our city, you will collaborate with partners across Health Organisations, Children services, Education and wider community partners.
As one of the leaders for the service, you will lead the way to ensure the best possible response for adults in our service. Based at Civic Centre with flexible working options, you will support practitioners to work across locality areas reaching out to community-based organisations.
