The role focuses on supporting older adults across clinic, home, and residential settings to maintain and improve independence, reduce functional decline, and achieve meaningful goals through evidence-based, outcome-focused interventions aligned with Commonwealth aged care programs.  You will have the opportunity to work across hospital avoidance, dementia care, and healthy ageing service streams, the Occupational Therapist delivers targeted reablement interventions that address a range of complex conditions including frailty, cognitive impairment, falls risk, and chronic health issues.
