This is a rewarding role ideal for a licensed clinician eager to make a lasting impact on the lives of underserved communities through integrated care.   As a School-Based Behavioral Health Provider, you will: * Deliver brief, solution-focused assessments and interventions aimed at functional restoration. * Collaborate closely with primary care providers to develop and implement clear treatment plans. * Provide behavioral health services, including crisis intervention and substance abuse screenings. * Educate and consult with the primary care team on mental health, chemical dependency, and behavioral medicine. * Support patients and families with self-management, behavior change, and harm reduction strategies. * Identify referral resources and coordinate care for patients requiring specialized services. * Maintain thorough documentation in Electronic Medical Records (EMR).   Key Responsibilities: * Conduct comprehensive bio-psychosocial assessments addressing mental health, substance use, co-occurring disorders, and medical needs. * Manage psychosocial aspects of chronic and acute diseases in collaboration with the primary care team. * Be immediately available during working hours to assist with crisis interventions and psychiatric emergencies. * Provide individual, group, and family interventions as needed within the school setting. * Use electronic tracking tools to monitor treatment outcomes and ensure effective care coordination. * Educate staff, patients, and families on mental health best practices.   Qualifications: * Master’s degree in Clinical Social Work, Counseling, or Psychology. * Licensed in the State of Ohio (LISW, LPC, LPCC, LMFT). * Three (3) or more years of experience in behavioral health, preferably in an integrated primary care setting. * Strong knowledge of population-based healthcare, behavioral medicine, and evidence-based treatment models. * Experience working with diverse, multicultural populations. * Bilingual (Spanish/English) strongly preferred.   Equal Employment Opportunity/Drug-Free Workplace:   The HealthCare Connection is focused on creating a community that promotes dignity and respect for employees, patients and other community members.
