We are looking for you to work as as part of the Aylesbury Rapid Response & Intermediate Care team supporting patients in their own homes , providing personal , social and rehabilitative care . The overall aims of the Rapid Response & Intermediate Care Team are to avoid inappropriate admission of patients into Hospital and facilitate timely discharge into the community.
The team is committed to ongoing education on the job and development opportunities are available within the organisation . Successful applicants who don’t already have a Level 3 Qualification in Health and Social Care will be expected to work towards that in post.

Providing general and specific rehabilitation focused care to adults, including personal care, exercise supervision, rehabilitation with domestic tasks and equipment delivery within competencies
Being an integral member of the Rapid Response & Intermediate Care (RRIC) team, providing defined delivery of nursing, social and rehabilitative care to patients , working in collaboration with senior staff.
Taking responsibility for designated elements of the patients care in patients’ own homes.
Being responsible for the completion of delegated non-clinical tasks to support the efficient functioning of the team.
