We are seeking a motivated and compassionate Unqualified Social Care Officer to join our Adults Social Care team.   About the Role   As an Unqualified Social Care Officer, you will work with adults, carers and families in the community to deliver person‑centred strength-based assessments, support planning, and reviews. You will manage an allocated caseload, and support people to achieve the outcomes that matter most to them.   A key part of the role includes working with young people aged 18–25 who are transitioning from Children and Young People’s Services (CYPs) into Adult Social Care. Experience of working with young people with disabilities and/or mental health needs is essential. An understanding of SEND, education pathways and Education, Health and Care Plans (EHCPs) is desirable.
