Service coordination means the activity of linking a project resident to needed supportive services or medical agencies in the general community.  Additionally, the term may cover case management, both formal and informal, in which the service coordinator assesses service needs, determines eligibility for public services, and makes resource allocation decisions. Essential Job Functions: * Provides general case management (including intake) and referral services to all residents needing such assistance.  * May provide formal case management (i.e., evaluation of health, psychological and social needs, development of an individually tailored case plan for services and periodic reassessment of the resident’s situation and needs) for a resident when such services are not available through the general community.   * Establishes linkages with appropriate agencies and service providers in the community; shops around to determine/develop the best “deals” in service pricing, to assure individualized, flexible, and creative services for the involved resident(s).  * Sets up a directory of providers for use by both project staff and residents.  * Refers and links the residents of the project to service providers in the general community.  There are, for example, case management, personal assistance, homemaker, meals-on-wheels, transportation, counseling, occasional visiting nurse, preventive health screening/wellness, and legal advocacy.   * Educates residents on service availability, application procedures, client rights, etc., providing advocacy as appropriate.  * Monitors the ongoing provision of services from community agencies and keeps the case management and provider agency current with the progress of the individual.  Manages the provision of supportive services where applicable.   * Helps the residents build informal support networks with other residents, family and friends.   * May set up volunteer support programs with service organizations in the community.   * May provide training to project residents in the obligation of tenancy or coordinate such training.  * May educate other staff on the management team on issues related to aging in place and service coordination, to help them to better work with and assist the residents.   * May develop case plans in coordination with community assessment services or with a PAC (Professional Assessment Committee).   * Maintains accurate records and files in most confidential manner.  This file will be locked and reviewed only by the Executive Director, Executive Director or CEO of the company, when appropriate and/or needed.   * Completes other duties as assigned by the Executive Director but cannot be assigned the following duties:  providing social activities; assessing residents’ level of functioning; providing transportation; providing medical services or advice; providing direct services or care; or any office/clerical tasks.   Other Job Functions: Models and embodies Eskaton’s core values, including:  * Creating a sense of warmth, welcoming and belonging for all. * Fostering excellence in everything we do. * Seeking consistent improvement, learning and sustainable growth. * Stepping up, taking ownership and leading with initiative  * Fully complies with all provisions in Eskaton’s Corporate Compliance Policy and the Health Insurance Portability and Accountability Act (HIPAA).   * Maintains attendance as outlined in Eskaton policy.  * Follows required dress code and Eskaton’s Community Appearance Standards.  Maintains a professional image to the public and residents * Works in accordance with established safety guidelines; uses proper body mechanics; follows safe work practices; wears appropriate safety gear.     Starting pay ranges from $22.90 - $30.30 per hour. Qualifications:   Education:  * A Bachelor of Social Work or degree in Gerontology, Psychology, or Counseling is preferable; a college degree is fully acceptable.  However, individuals without a degree, but with appropriate work experience, may be hired.  Training and Experience:  * Two years' experience with progressive responsibility in management functions.   Knowledge and Abilities:  * Demonstrated working knowledge of supportive services and other resources for senior citizens and non-elderly disabled in the area served by the project.   * Demonstrated ability to advocate, organize, and problem-solve with proven results for the elderly and disabled served.   Work Conditions & Physical Requirements Requirements are subject to possible modification to reasonably accommodate individuals with disabilities.
