What You'll Do Screen patients and families for social determinants of health and connect them with community resources, healthcare services, and support programs. Serve as COPA's representative in the community by building relationships with local organizations, schools, and community partners to strengthen access to care. Independently manage referrals, patient follow-up, and family navigation while exercising sound judgment and initiative to remove barriers to care. Educate and advocate for patients and families by coordinating services, providing culturally responsive support, and helping families successfully navigate healthcare and social service systems. Document patient interactions, monitor outcomes, and collaborate with providers and care teams to improve health equity and deliver exceptional, family-centered care. Assist patients and families with Oregon Health Plan (OHP) enrollment and insurance applications by providing education, completing applications, managing follow-up, documenting activities, and serving as a liaison between patients and state agencies to ensure timely access to coverage. OHA Certified Assister certification is preferred. OHA approved Community Health Worker certification preferred upon hire. Certification is required within first year of employment and employee must complete an OHA approved CHW certification course. Experience Pediatric Community Health Worker experience preferred with excellent communication, strong attention to detail, and exceptional time management skills. Ability to work independently and take initiative while collaborating effectively with multidisciplinary teams; strong analytical, problem-solving, and decision-making skills. Knowledge of Patient-Centered Primary Care Home (PCPCH) and care coordination principles preferred; clinical informatics experience is a plus. Epic EMR experience preferred and proficiency with Microsoft Office Suite required. Bilingual in Spanish and English is a plus. Working Conditions Ability to perform the physical requirements of the role, including prolonged sitting or standing, frequent computer use, walking, and occasional lifting of up to 30 pounds (rarely up to 50 pounds). Ability to wear appropriate personal protective equipment (PPE) as required. Works primarily in a medical office setting with eligibility for a hybrid schedule in accordance with COPA's Local Remote Work Policy. Ability to travel between COPA clinics, the business office, and community partner locations as needed.
