Identify appropriate services or placements following discharge and ensure timely, effective referrals (housing, outpatient care, substance use treatment, etc.). Advocate for clients’ rights and ensure their voice is heard throughout their care journey. Maintain up-to-date and accurate documentation of all case coordination efforts in compliance with HIPAA and KDADS regulations. Work to remove common barriers to discharge such as lack of housing, transportation, or insurance. Support the development of client-centered tools like WRAP (Wellness Recovery Action Plans).
