Community Healthcare of Texas has provided hospice and palliative care services since 1996.  Our mission is to provide compassionate care for individuals living with serious illnesses while supporting their loved ones who care for them. Community Healthcare of Texas is currently recruiting for an RN Case Manager. POSITION SUMMARY   Meet with patients and families to explain the scope of hospice services, coordinate admission of patients, and ongoing plan of care.   ESSENTIAL FUNCTIONS * Provide professional nursing care by utilizing all elements of nursing process * Assess and evaluate patient’s status by writing and initiating plan of care. Regularly re-evaluating patient and family/caregiver needs, and participating in revising the plan of care as necessary * Develop a care plan that establishes goals, based on nursing diagnosis and incorporates hospice nursing actions. Include the patient and the family in the planning process * Administer medications and treatments as prescribed by the physician * Provide healthcare instruction to the patient as appropriate per assessment and plan * Act as Case Manager when assigned by clinical Supervisor and assume responsibility to coordinate patient care for assigned caseload * Complete, maintain, and submit accurate and relevant clinical notes regarding patient’s condition and care given. Record pain/symptom management changes/outcomes as appropriate * Communicate with the physician regarding the patient’s needs and reports changes in the patient’s condition, obtains/receives physicians’ orders as needed * Communicate with community health related persons to coordinate the care plan * Provide and maintain a safe environment for the patient * Work in cooperation with the family/caregiver and hospice interdisciplinary group to meet the needs of the patient and family/caregiver * Attend interdisciplinary group meetings * Participate in on-call rotation; may assist with admits * Manage assigned patients equally, regardless of severity, contagion, or complexity of diagnosis * Supervise LVN and CNA care plan and delegate responsibilities when required.
