As an Admissions RN, you’ll guide patients and their loved ones through the start of their hospice journey—completing initial assessments, providing skilled nursing care, and helping them understand what to expect. You’ll collaborate with the full care team to ensure a smooth, thoughtful transition and a personalized plan of care from day one. More than just opening a chart, you’ll open the door to peace, support, and comfort—meeting people where they are and setting the tone for the sacred season ahead. Your Role in the Mission As an Admissions Nurse at Lifted Hospice, you are often the first clinical voice a patient and family will hear. You guide them through the transition into hospice with clarity, compassion, and confidence. Your role is both clinical and relational, ensuring each patient is eligible, informed, and emotionally supported as they begin care. You represent The Lifted Way in action, making sure patients are welcomed with dignity, families feel seen and heard, and every admission starts with excellence. Key Responsibilities Include: Assessing needs – Completing timely and thorough nursing assessments to identify each patient’s physical, emotional, and environmental needs at admission. Building the plan of care – Developing and documenting a personalized care plan in coordination with the patient, family, and interdisciplinary team. Providing skilled care – Delivering compassionate, clinically excellent nursing care at the start of care, using proper infection control measures in line with OSHA standards. Working as one team – Collaborating closely with the medical director, physicians, hospice leadership, and the full interdisciplinary team to ensure patient and family needs are fully understood and supported. Keeping clear records – Submitting all required documentation and admission paperwork accurately and on time, maintaining compliance with all regulatory standards. Supporting families – Offering education, comfort, and clear communication to help families understand the hospice journey and feel confident in what’s ahead. Staying ready to serve – Participating in on-call rotations and providing patient care and support after hours as assigned. Growing in excellence – Participating in ongoing training, hospice in-services, and quality improvement (QAPI) initiatives to maintain clinical excellence and professional growth. Skills and Qualifications Graduate of an accredited Diploma, Associate, or Bachelor’s nursing program, with an active state license and clinical skills that meet current standards of nursing practice. At least two years of experience as a registered nurse in a clinical care setting. Hospice experience preferred. Strong assessment, critical thinking, and communication skills — able to guide urgent and emotional conversations with calm and clarity. Familiarity with state and federal hospice regulations (including eligibility criteria and CoPs). Ability to work independently, prioritize needs quickly, and make decisions under pressure. Comfortable navigating emotional, logistical, and cultural complexities during admissions. Able to meet documentation deadlines while supporting patient and family needs. Must be able to travel locally to complete admissions across homes and facilities. Current driver’s license, reliable transportation, and valid auto liability insurance. Must be able to read, write, and communicate clearly in English. Where You’ll Work In this role, you’ll spend most of your time meeting patients and families wherever they call home—whether that’s a private residence, memory care facility, or other long-term care facility. You’ll help them take the first step into hospice care with clarity, comfort, and confidence. You’ll collaborate closely with your local team of nurses, social workers, chaplains, physicians, and our medical director to ensure each patient is admitted with a clear, compassionate plan that reflects their goals and needs. While your role focuses on admissions rather than ongoing visits, you play a vital part in setting the tone for the full Lifted Hospice care experience. This role requires local travel, flexibility, and the ability to adapt to each unique setting with professionalism, presence, and compassion.
