Emory Healthcare’s Telehealth Nursing team is unable to guarantee you will have the same weekday off. Description Job Summary: The LPN Ambulatory Virtual/Telehealth Nurse will provide patient care using virtual platforms, demonstrating proficiency in telehealth technologies and clinical protocols. They will assess and manage patient needs, collaborate with providers, manage prescriptions, and complete prior authorizations. Strong time management, attention to detail, and cultural sensitivity are crucial in this role. Key Responsibilities: Patient Care & Clinical Management: 1. Provide virtual nursing care using telecommunication technologies (telephone, video, chat, etc.). 2. Assess patient needs using clinical protocols, guidelines, and Synergy model for Care. 3. Assist with medication management and scheduling appointments. 4. Route medication renewal orders to providers for review per protocol. 5. Collaborate with patients regarding prescription management, including protocol orders. 6. Identify and report patient safety and quality issues. 7. Act as an advocate for the patient, supporting optimal health outcomes. 8. Educate patients on the provider's plan of care. 9. Resolve pharmacy/medication issues in collaboration with providers. Operational Support: 1. Document, update, and maintain patient medical records. 2. Support clinical and administrative operations of telehealth. 3. Participate in staff meetings, huddles, and organizational meetings. 4. Train, orient, and educate new team members and students. 5. Maintain confidentiality in all interactions and follow telehealth policies. Communication & Collaboration: 1. Serve as a liaison between patients and providers. 2.
