Description The Physical Therapist II serves as a primary clinician, providing comprehensive physical therapy evaluations, treatments, and education to patients with complex medical and surgical conditions. Key Duties and Responsibilities: Patient Care & Treatment: Conducts comprehensive evaluations, testing, and treatment for patients with complex medical and surgical conditions. Educates patients and their families in a manner tailored to their needs. Develops and adjusts individualized treatment plans with clear, measurable goals. Ensures daily practices align with established patient care standards. Recommends appropriate equipment for patient care. Assists other staff in assessing patients and problems. Collaboration & Communication: Communicates effectively with patients, families, and the healthcare team. Participates in interdisciplinary team conferences to enhance patient care. Works cooperatively with other team members to maintain a positive and results-oriented work environment. Collaborates with care teams and community resources for post-discharge planning and follow-up. Responds to requests for professional information or advice, referring complex matters to appropriate personnel. Participates in quality assurance activities. Monitors and ensures compliance with infection control and sterile technique protocols. Documents patient care details and records service charges accurately. Manages inventory, equipment maintenance, and work area organization. Serves as a Clinical Instructor for short term and long-term students and may assist with Residents. Assists with new staff orientation. 8. Assists department in maintaining a clear and accurate interdisciplinary and interdepartmental information flow. Professional Development & Compliance: Attends relevant educational in-services and professional activities to stay current on industry practices. Works with the Leader to develop a professional growth plan. Delegates tasks effectively as appropriate. Adheres to all applicable policies, protocols, and regulations.
