Review and assess all medication orders to identify, prevent, and resolve drug-related problems. Conduct drug utilization reviews and evaluate interactions for every prescription. Provide clear, accurate medication counseling and education to patients and staff. Maintain designated weekly clinical hours outside the pharmacy to participate in care team meetings and interdisciplinary discussions. Serve as the primary point of contact for pharmacy-related concerns and operational issues. Perform monthly audits, including documentation review, inventory management, and 340B compliance monitoring. Collaborate effectively with physicians, nursing staff, leadership, and external partners. Prepare operational and compliance reports as assigned.
