Essential Job Functions
Assist individuals in all areas of personal hygiene, activities of daily living and care of their living area and personal belongings. When possible, encourage the highest practicable level of independence. [1]
Provide treatment and determines and administers medications by following the “Five Rights,” of right resident, right medication, right dosage, right time, and right route/method.
Assist individuals at mealtime including food prep, feeding and supervision while dining. Ensures each individual diet is followed. [1]
Provide supervision of individuals to always maintain safety. Maintain a safe, clean environment free of hazards. After meals, assure that dining area floors, tables, chairs and wheelchairs etc. are cleaned of food and liquids. [2]
Follow directions and perform care as assigned and report progress with those assignments. Document care provided to individuals in their chart. Participate in defining personal needs to assist in development of an individualized service. [3]
Follow all individual service plan approaches including those developed by other disciplines including Nursing Services, Physical, Speech, or Occupational Therapy. [2]
Gather and document appropriate data on individuals such as specific behaviors. Immediately report changes in physical and mental condition to the registered nurse for appropriate direction and intervention. [3]
Conscientiously observe assisted living rules, regulations and policies. [1]
Perform duties such as temperature, pulse, respiration, blood pressure, weights, medication administration and treatments according to individuals ISP and under the general supervision of a registered nurse as allowed under regulation and NP Act. [1]
Coordinate and accompany individuals to worksites, appointments, and a variety of community events. Drives facility vehicles in the transportation of individuals to community events, medical or dental appointments etc. [3]
Provide treatment and medication administration to individuals. [1] Maintain resident rights and confidentiality of resident information. [3]
Immediately report all suspected cases of abuse or neglect to the Assisted Living Supervisor or Administrator. [3]
Work safely, following the safety practices and standards of the organization including Blood Borne Pathogen Standards. Uses a gait belt and proper body mechanics when involved in lifting, pushing or pulling activities. [1]
Changes bed linens, collects laundry and assists with housekeeping duties which may include vacuuming, dusting and sweeping/mopping. Empties garbage regularly. Assures areas are neat and safe. [1]
Works with supervisor to implement planned recreational and social programs and to engage individuals. [1]
Participates in and completes in a timely manner, in-service and training programs.[1]
Answer phones and directs calls, greets visitors and performs some clerical duties as necessary. [2]
Employee is expected to report to work when scheduled, with minimum use of sick leave, to build up sick leave balance for income protection for unexpected illness. [3]
In this public service position, employee is required to be courteous, cooperative and respectful at all times, with the public and clients. Also establishes and maintains a courteous, cooperative and respectful working relationship with other employees, supervisors and public officials.
Physical demands: Uses near vision, stands, walks, and moves around the facility 75% of shift. Uses hands for delivering personal care and treatments. Communicates with residents and other staff members using talking and hearing. Read and write English. Ability to kneel or squat within six inches of the floor, pushes and pulls carts, wheelchairs with residents weighing up to 250 pounds. Lifts 35 pounds, transfers up to 50 pounds. Uses nursing-related equipment such as blood pressure device, scales, mechanical lifts and adaptive/restorative equipment.

Knowledge, Skills and Abilities

Ability to work effectively with a variety of individuals including individuals with dementia, mental illness or developmental disabilities.
Understand and follow oral or written directions.
Training, Experience and Other Requirements
Experience as a Resident Care Assistant in a Residential Care Apartment Complex (RCAC) or Community Based Residential Facility (CBRF) is preferred.
If classified as a Certified Nursing Assistant (CNA), the employee must be listed on the Wisconsin Nurse Aide Registry.
For those working in a Community Based Residential Facility (CBRF), the employee must be listed on the Wisconsin CBRF Employee Registry as having completed the four core DHS 83 courses—Standard Precautions and Medication Administration (prior to related duties), and Fire Safety and First Aid & Choking (within 90 days of hire).
Prefer experience working with geriatric, mentally ill or developmentally disabled persons.
Employees driving County vehicles must have and maintain a valid Driver’s License and a good driving record.
Must successfully pass a background check as required by the Wisconsin Caregiver Law.
Must be 18 years of age.
