The Transport Aide / Certified Nursing Assistant (CNA) is responsible for providing high-quality, compassionate care to residents while supporting their medical and personal needs both on-site and off-site. This dual-role position functions as a CNA on the unit as needed and also accompanies residents to medical appointments, assisting them throughout the visit and safely transporting them using the facility van. Transport Aide, accompanies residents to medical, dental, and other off-site appointments as scheduled. Assists residents before, during, and after appointments to ensure comfort, safety, and understanding of care instructions. Safely transports residents to and from appointments using the facility van in compliance with traffic laws and facility policies. Assists residents with entering and exiting the vehicle and secures wheelchairs and mobility devices properly. Communicates appointment outcomes, instructions, and observations to nursing staff upon return. Maintains cleanliness and safety of the facility vehicle and reports any maintenance or safety concerns promptly.
