Vos responsabilités Sous l’autorité hiérarchique du Cadre de secteur/santé, vous avez pour missions sur votre périmètre d’intervention de : Assurer les soins d’hygiène et de confort des patients sous la responsabilité et le contrôle de l’infirmier. Apporter votre aide dans l’accomplissement des gestes quotidiens. Développer auprès du patient et de son entourage la dimension préventive, relationnelle et éducative du soin dans sa visée d’autonomisation. Assurer les transmissions orales au cadre de secteur/santé, à l’équipe pluridisciplinaire au sein de l’HAD et aux autres intervenants externes. Participer aux réunions d’équipe. Aider au choix du matériel de soins en prenant en compte la maîtrise des coûts ; Sensibiliser le patient et son entourage à l’utilisation adéquate du matériel ; Vous contribuerez également à l’accueil et l’intégration des nouveaux collaborateurs, aux partages de vos savoir-faire et compétences auprès de vos collègues. Vous avez Diplôme d’Aide-Soignant requis avec un permis B. Expérience de soins à domicile souhaitée. Connaissance des outils informatiques. Capacités à analyser les situations et en évaluer l’urgence, à travailler au sein d’une équipe pluridisciplinaire.
