POSITION OVERVIEW:

The Registered Practical Nurse (RPN) in Long-Term Care provides compassionate and patient-centered care to residents in a long-term care facility. The RPN works as part of a collaborative healthcare team to deliver nursing care, monitor patient conditions, and support the overall well-being of residents, many of whom may have chronic conditions, cognitive impairments, or physical disabilities. The role requires clinical expertise, empathy, and the ability to handle complex care needs while maintaining a supportive and respectful environment for both residents and their families.

QUALIFICATIONS:

• Education:
• Diploma in Practical Nursing from an accredited program.
• Licensure:
• Current registration with the College of Nurses of Ontario (CNO) (or applicable regulatory body for your location).
• Experience:
• Previous experience in long-term care, geriatrics, or a related field is an asset.
• Experience with dementia care, palliative care, or complex medical conditions is preferred.
• Skills and Abilities:
• Strong clinical assessment and critical thinking skills.
• Ability to provide empathetic and patient-centered care.
• Excellent communication skills, both verbal and written.
• Ability to work effectively in a multidisciplinary team environment.
• Knowledge of electronic health records (EHR) and basic computer literacy.

• Resident Care:
• Perform nursing assessments and monitor resident conditions, noting any changes in health status.
• Administer medications, treatments, and interventions as per care plans and physician orders.
• Provide wound care, dressings, and other clinical procedures.
• Assist in the development and revision of individual care plans for residents.
• Ensure comfort and dignity for residents during daily activities, assisting with personal hygiene, mobility, and feeding as needed.
• Clinical Support:
• Document and maintain accurate records of all care provided, including changes in resident health and medications.
• Monitor vital signs, report abnormal findings, and implement appropriate interventions.
• Assist in coordinating resident care with physicians, specialists, and interdisciplinary care teams.
• Participate in resident admissions, discharges, and transfer processes.
• Family and Resident Communication:
• Communicate with residents and their families to provide updates on care progress and address any concerns.
• Educate residents and families about health conditions, treatment plans, and care routines.
• Team Collaboration:
• Collaborate with Registered Nurses (RNs), physicians, personal support workers (PSWs), and other healthcare staff to provide holistic care.
• Act as a role model and mentor to junior nursing staff and PSWs.
• Participate in team meetings, including care conferences and quality improvement initiatives.
• Health and Safety Compliance:
• Follow all health and safety guidelines, including infection control practices, to ensure a safe and clean environment.
• Adhere to facility policies and provincial regulations regarding resident care and nursing practice.
• Professional Development:
• Maintain current RPN licensure and stay updated on best practices in long-term care nursing.
• Attend training sessions and educational workshops as required.
