Our Nurse Practitioner Department is renowned for its teamwork, and you’ll have the opportunity to shape and develop this new role, contributing to the ongoing evolution of the Transitional Care program. This is a Post-Graduate Advanced Education (PGAE) program opportunity—a six-month initiative designed to attract, train, inspire, and retain outstanding acute care Nurse Practitioners who provide exemplary care to a diverse patient population. The program features structured onboarding with supervised clinical training across a range of acute care specialties. Didactic sessions are delivered in an interdisciplinary setting and focus on core competencies in acute care, clinical reasoning, simulation, work-life balance, and quality improvement. Throughout the program, NP participants will receive regular follow-up and dedicated support from an Acute Care NP Lead to ensure a smooth and successful transition into their role. A minimum 18-month return-of-service commitment is required upon completion of the PGAE program. Participants are expected to demonstrate a strong commitment to their clinical position and to the specialty, with the intention of contributing as subject matter experts to the growth and sustainability of future PGAE programs. As a Nurse Practitioner with VCH you will: Be responsible and accountable for the comprehensive assessment of patients and clients including diagnosing diseases, disorders and conditions. Initiate treatment including health care management, therapeutic interventions and prescribes medications in accordance with the statutory and regulatory standards, limits and conditions, and employer policies and procedures. Provide professional guidance to other health professionals and participates autonomously as a member of the interdisciplinary health teams. Collaborate with clients and other health professionals to identify and assess trends and patterns that have implications for clients, families and communities. Develop and implement population and evidence-based strategies to improve health and participate in policy-making and quality improvement activities that influence health services and practices. Participate in peer, program and self-review to evaluate the outcome of the services at the client, community and population level.
