Tez Healthcare Services is currently seeking a Licensed Practical Nurse (LPN) to provide in-home care services in the community. This is an opportunity to work in a supportive and flexible environment focused on client-centered care.

SKILLS:

  • Experience working in medical office or hospital settings
  • Strong patient care skills with the ability to assess, plan, implement, and evaluate nursing interventions
  • Knowledge of medication administration procedures and ability to safely administer medications

Responsibilities
  • Provide direct patient care to a senior client at his home.
  • Administer medications and treatments as prescribed by healthcare professionals
  • Monitor patient vital signs and report any changes or concerns to the appropriate medical staff
  • Assist with patient assessments and develop care plans in collaboration with the healthcare team
  • Perform routine procedures such as wound care, catheterization, and specimen collection
  • Document patient information accurately and maintain medical records
  • Provide emotional support and education to patients and their families
  • Collaborate with case managers to coordinate patient care and ensure continuity of services
