With a focus on dignity, prevention, and collaboration, the centre supports aging in place and reduces barriers to care for vulnerable seniors in Calgary.   Reporting to the Clinic Manager, The RN works to full scope of practice through the application of knowledge to “assist clients in meeting their health needs in whatever setting, complexity and situation they occur throughout the life span. Scope of practice includes all the interventions that registered nurses are authorized, educated and competent to perform. It is limited by the specific competencies of the individual registered nurse to perform the activities necessary for the client population with whom that nurse practice, within applicable legislation and requirements of the employers as described in employer policies.” — College and Association of Registered Nurses of Alberta, Scope of practice for Registered Nurses, 2011.   This is a full-time, contract position until November 3, 2026, with the possibility of extension.    What You Will Do * Chronic Disease Management: Assess and support chronic disease management, guiding and empowering clients in managing their health.  * Tobacco and vape cessation education and non-judgmental support providing referrals, education, and linkages to programs for patients interested in cessation. * Mental health and addiction screening and providing support, education, resources, and referral when appropriate. * Reproductive Health: Provide inclusive, age-appropriate care related to contraception, menstrual health, STIs, and healthy relationships. Offer confidential support and make referrals when needed.  * Complete appropriate health questionnaires e.g. MOCA, ASQ, PHQ-9, GAD-7, Vanderbilt to assess mental and physical health needs.  * Best possible medication history: Works with clients to create a complete, accurate, and up-to-date medication list. * Advanced Care Planning: Provide guidance and clarity to clients and families as they plan for future health needs.  * Quality Improvement: Develop an understanding of the principles of quality improvement, apply quality improvement principles in the provision of care, participate and initiate the implementation of improvement processes within primary care that support improving patient care. 1. Collaborate effectively with the multidisciplinary team to achieve change and improvements in patient care. 2. Support and mentor team members and colleagues in the principles of quality improvement. 3.
