Description

Floor Nurse Day Shift and Night Shift (Night Shift has $2,500 sign-on bonus) - schedule options available: Weekend Option, PT, FT.
1. To provide leadership and direction in accordance with the facility’s policies and procedures to meet the goal of quality nursing care.
2. To provide for the growth and development of the shift personnel by identifying needs and seeing that they are met.
3. To provide for the safety and comfort of residents during the shift.
4. To provide for the safety and health of personnel assigned to the shift.
5. To monitor utilization of equipment, supplies, and staff during the shift.
6. To promote open communication within the shift and with other departments.
7. To assist with the practice of emergency procedures held during the shift.
8. To supervise all nursing activities related to nursing care.
9. To set standards for accurate reporting and recording of the resident’s symptoms, treatments, reaction, and progress.
10. To organize resident care to ensure continuity of care based on knowledge of resident care plan needs.
11. To evaluate resident care through assessment, planning, intervention, evaluation, and written documentation.
12. To administer nursing care to residents.
13. To assess and strive to meet resident needs.
14. To assess resident/family status to assist in the planning of resident care.
15. To communicate with family and health care professionals regarding care of the resident.
16. To provide for the professional growth and development and establish a climate conducive to meeting educational needs of staff, family, and visitors.
17. To interpret goals, objectives, policies, and procedures for all staff, resident, and family.
18. To continue own education by attending inservices and seminars designed to provide the best possible care for residents.
19. To foster good public relations for the nursing department and the facility.
20. To conduct self in a mature, professional manner to influence the action of other staff members.

PREFERRED QUALIFICATIONS:

  1. Experience in long-term care facility
  2. Current and up-to-date CPR card from American Heart Association or Red Cross
  3. Any specialized training and/or certificates in geriatrics, intellectual disability, or any special needs
  4. IV training certificate
  5. Current and up-to-date TB card from the Indiana American Lung Association
Responsibilities

ESSENTIAL FUNCTIONS:

  1. Document in the resident’s medical record all pertinent charting as delineated in the policies and procedures.
  2. Plan and evaluate care to meet the resident’s needs using the nursing process which includes planning, implementation, and evaluation.
  3. Complete assessments as required on assigned residents.
  4. Notify the attending physician and family of any incidents or change in the resident condition.
  5. Transcribe physician orders verbal, telephone, and/or written orders into the computer program and include all components necessary for specificity.
  6. Administer and document administration of medications both routine and as needed per physician order into the computer program.
  7. Perform procedures both invasive and non-invasive including, but not limited to, urinary catheterization, insertion and maintenance of an intravenous line, enemas, maintenance of colostomies, operation of a bladder scanner, maintenance and care of gastric tubes, and other procedures for which training and orientation were completed.
  8. Perform treatments per physician orders and assess areas being treated.
  9. Receive report form nurse on previous shift and give report to nurse coming on duty prior to leaving.
  10. Document information to be passed on in report on the 24-hour report sheet. Information must include all physicians’ orders received during the shift, all changes in resident condition, any information obtained from the family, any transfers, discharges, leave of absence, or other pertinent information.
  11. Notify the appropriate laboratory of any scheduled labs that were ordered during the shift.
  12. Notify the physician of any abnormal lab results received via telephone or delivered lab reports.
  13. Complete the admission process as necessary and supervise the CNA during the admissions process.
  14. Complete the discharge process including home instructions if needed which must be in easily understood layman’s terms.
  15. Notify respiratory therapy as needed if respiratory treatment was ordered by physician. Also notify therapy of any new therapy orders.
  16. Notify the oxygen provider of need for more liquid oxygen or concentrators.
  17. Notify the Administrator, Director of Nursing, the Dietary Department, the Office Manager, the Activities Director, Social Service, and Maintenance/Laundry/Housekeeping Supervisor of any resident who is deceased, transferred, admitted, or taken a leave of absence during the shift.
  18. Notify the Dietary Department as soon as possible of any new admission’s diet order or of any resident who will not be present during meal time, require a meal either earlier or later than the resident’s regular meal time, or will be served the meal in room or other area resident does not usually consume meal.
  19. Make every effort to replace any personnel who have indicated they will not be present for work. If the Scheduler or the Director of Nursing is present, notify them immediately of call-in status. Complete an employee absentee form and turn it into basket in central supply room.
  20. Notify the pharmacy of any new medication/treatment orders or change in medication or treatment orders if required by computer program.
  21. Document properly on any controlled substances administered during the shift. Complete and document the narcotic count prior to receiving and giving report. Always complete this with the nurse coming on duty or nurse going off duty.
  22. Receive and verify receipt of medications and treatments from pharmacy.
  23. Supervise the CNAs and QMAs assigned to the shift. Assist as needed in providing general care to the resident. Monitor and direct QMAs and CNAs as needed.
  24. Complete, direct, and/or participate in emergency measures or drills.
  25. Educate staff, residents, and family as necessary.
  26. Assist in maintaining a safe, clean environment by reporting equipment which is out of order and completing assigned cleaning tasks.
  27. Observe and assess residents for any change in condition or behavior and follow-up with notification of family and physician. Document actions in the computer program.
  28. Answer call light as needed.
  29. Transport residents to various areas of facility as needed.
  30. Monitor and assist in dining room during resident meals as needed.
  31. Perform or withhold cardio-pulmonary resuscitation (CPR) in a way consistent with the policies and procedures of this facility.
  32. Take and record or delegate the taking and recording of vital signs (blood pressure, pulse, temperature, respirations). Report to physician any results not consistent with resident’s condition.
  33. Collect and prepare for pick-up any laboratory specimens ordered by physician.
  34. Establish and maintain good interpersonal relationships with residents, family, visitors, or other staff while ensuring confidentiality of resident information.
  35. Properly complete any assigned documentation.
  36. Notify the Administrator or Director of Nursing of any situation that cannot be properly be handled by on-duty staff.
  37. Discontinue and document properly all destruction of medications except controlled substances. At least one Nurse Manager is required to participate in the destruction of controlled medications.
  38. Notify the Administrator immediately of any abuse or allegations of abuse regarding residents and follow the abuse policy of this facility.
  39. When antibiotics are ordered by physician, obtain and document reason for antibiotic and duration of antibiotic. If any labs, cultures, or x-rays were obtained related to the infection for which the antibiotic is ordered, ensure physician notification if such results.
  40. Any other duties as deemed necessary by the Administrator or designee.
