Description

We are looking for a dynamic, enthusiastic, resilient and dedicated Clinical Lead Nurse Manager to join our Integrated nursing and therapy Urgent Community Response team in South Norfolk, to support with both the daily clinical and operational staff management. This is a fast-paced but rewarding role that would suit an experienced nurse who has a background in acute, community or significant experience in other urgent and emergency response teams.
This role will provide an opportunity for new challenges and on-going development in a multi-disciplinary innovative, supportive and friendly team who work closely with other community and NHS services to prevent admission to hospital.
The successful candidate will have excellent communication skills and work collaboratively to meet the needs of the service. It is essential that you have evidence of professional competencies, and are able to effectively apply your knowledge and skills to clinical practice and share your knowledge with the team in a range of ways.
Job Purpose
T o provide clinical expertise, clinical leadership and line management to staff working within Urgent Community Response (UCR).
To support staff to achieve and ensure competencies are maintained within the services
To contribute in the delivery of high standards of health care to patients within their own places of residence
To ensure that Trust annual priorities and quality goals are embedded within the workforce by ensuring that all annual appraisals are complete within the team.
Hours of Operation
The service operates between 08:00 and 20:00 seven days a week and you will be required to work shifts between these times.
The Urgent Community Response (UCR) Team cover all of South Norfolk, so you will be expected to support staff working across the South Norfolk area.
You will positively contribute to service and quality development through new ways of working, and will work alongside the Clinical Operations Manager and UCR nursing and therapy Leads to support with operational issues and professional development of staff and the service.
The UCR team provide admission avoidance services via the Rapid Response MDP (formerly NEAT), 2 hour UCR response referral process, and referrals for end of life patients and the discharge to assess pathway.
UCR is participating in current and future innovative projects to support the on-going need to keep patients out of hospital and care for them at home. You will be expected to contribute in supporting Virtual Ward patients and the recently formed award winning Unscheduled Care Coordination Hub.
