Job Summary: The Nursing Care Program Coordinator role is part of the dispensary team that supports ETS’ mission through promoting continuity of care to improve patient care outcomes and improve access to care. This role makes a critical difference at ETS by coordinating patient care. The position is full-time onsite, and the days and hours are Monday to Friday 5:00 am to 1:30pm. What you contribute to this role – Responsibilities: Help ETS succeed in carrying out our mission through working together with other staff to transform systems of harm and inequity to create different approaches to improving community health and safety through addressing substance use and homelessness. Assist with the organization-wide initiative to reimagine our interconnectedness within our community to overcome the aspects of our society and organizational culture shaped by white supremacy and settler colonialism. The nurse coordinator will provide oversight and organization of the daily communication and coordination of care with other agencies, departments, and medical providers to provide optimum care to the patients, families, and medical/counseling teams. They will assist in coordinating nurse education, nurse onboarding, and other nursing functions as needed. Job Functions (May Include) Patient Care Per the Washington State Board of Nursing: The Licensed Practical Nurse (LPN) scope of practice in the nursing process is limited and focused. The LPN practices in an interdependent role when carrying out nursing care and a dependent role when carrying out nursing and medical regimens under the general direction of an authorized health care practitioner: Registered Nurse (RN), Licensed physician (MD), physician assistant (PA), advanced registered nurse practitioner (ARNP). Dispensing Medications: Prepares and dispenses medications as ordered by a medical provider; evaluates and documents the effectiveness of medications and any adverse reactions. Nursing Care: Provide direct nursing care using the nursing process, ensuring a compassionate and supportive experience for all patients. Collaborate with the Medical Director, Associate Medical Director, or Medical Providers for patient care, medication orders, or clarification. Patient Education: Promote health, wellness, and provide patient education. Utilize verbal and physical de-escalation techniques to manage behaviorally escalated/aggressive patients. Perform Testing services: Breathalyzers, Drug Testing Recordkeeping and patient confidentiality: Maintain accurate documentation in the electronic medical records (EMR) system.? Callbacks: Complete patient medication callbacks Person-centered care: Through promotion of shared decision-making between the patient and health-care provider while reinforcing respect for the patient’s values and preferences as they make decisions about their treatment. Integrate culturally responsive care and inclusiveness. Collaboration: With both internal and external multidisciplinary teams to provide nursing consultation and guidance regarding the health needs of potential and current patients. Trauma Informed Care: Promote person-centered and trauma-informed care grounded in the use of non-stigmatizing principles and language. Infection Prevention: Strict adherence to universal or standard infection control precautions as promoted by the Center for Disease Control and Prevention. Patient Emergencies: Follow ETS policies and procedures which outline how to respond to patient emergencies, including when to contact 911 for emergency medical services and when to contact 988 or local crisis numbers for mental health or substance use-related crises. Patients’ Rights and Informed Consent: Be familiar with informed consent policies at ETS to address any patient questions during the delivery of patient care. Informed consent supports treatment by ensuring patients have a clear understanding of medication side effects and risks, patient responsibilities, and ETS policies. All ETS staff will promote a sense of safety among patients and ensure that they are treated with dignity and respect. Patient Care Coordination Provides care coordination with external agencies and facilities. This includes chain of custody dosing, courtesy dosing, jail dosing, hospitalizations, dose verifications. Identify patients for care coordination in collaboration with the nursing, medical, clinical, intake teams and provide appropriate follow-up. Functions as the point of contact between Evergreen Treatment Services and all other external agencies where nursing care coordination is required. Terminate care coordination for the patient when it is no longer indicated. Promptly document service notes for all patient care coordination services provided. Promote strict confidentiality and the right to privacy when communicating with other agencies. Chain of Custody Procedures Functions as the point of contact within the organization to provide guidance and expertise on chain of custody processes to Evergreen Treatment Services Staff. As needed delivery of medications to facilities with established service agreements with ETS in consultation with the Program Sponsor, Associate Director of Nursing, Director of Nursing, and Medical Directors. Manage patient census reports of patients on a chain of custody dosing plan. Establish and/or maintain collaborative relationships with all external agencies who are involved in chain of custody dosing plans with Evergreen Treatment Services (including prospects). Prepare monthly status reports and provide updates regarding chain of custody procedures, census, etc. Actively identify and manage chain of custody issues as they arise and coordinate with appropriate personnel and external agencies. Maintain a current list of point of contact information for all external facilities Manage documentation around processes such as chain of custody records, ROIs, courtesy dosing paperwork. Develop chain of custody policy and procedures. Provide training to ETS staff and external agencies around chain of custody dosing procedures. Dispensary Operations Maintain day-to-day responsibilities in the dispensary (as needed). Dispense medications safely and accurately, ensuring accountability for inventory during each shift. Assist with daily dispensary operations, including preparation, organization, and compliance with all regulatory standards. Float between South King County Clinic and South Sound Clinic and associated mobile medication units as needed. Maintain accurate documentation in the electronic medical records (EMR) system. Participate in audits and reviews to ensure compliance and quality of care. Manage Callbacks Support the mission and teams with all other duties as assigned. Participate in regulatory audits including DEA, DOH, PQAC, SAMHSA and others. Complete QSTATIM follow up reports and investigations, including attending care team meetings, implementing corrective action plans. Participate in departmental and interdepartmental meetings and provide updates. Participate in agency interviews as assigned. Provide staff training as assigned, including new hires.
